摘要：城镇企业职工医疗保险是我国医疗保障事业的重要组成部分。近年来，我国城镇企业职工医疗保险改革取得了很大的进展。但是从苏北地区淮安、宿迁两地的实际调查情况看，经济欠发达地区城镇企业职工医保仍然存在不少突出的矛盾和问题。城镇企业职工医疗保险改革必须高度关注异地就医问题，加强医疗保险的宣传与监督作用，建立并完善社会保险和商业保险并行的医疗保障体系等等，这样才能提高城镇职工医疗保险制度的水平，解决城镇职工医疗中的难题。

一、城镇企业职工医疗保险制度的建立
城镇企业职工医疗保险是我国社会医疗保险的重要组成部分，其目的是为企业劳动者在医疗方面提供保障，为患病的企业职工提供经济上的帮助。新时期我国城镇企业职工医疗保险制度是逐步建立起来的，其发展过程可以分为城镇企业职工劳保医疗时期、城镇企业职工医疗保险时期两大历史时期，而城镇企业职工医疗保险又可分为试点、普遍实施以及新一轮改革启动三个阶段。
1。1949年——1994年：城镇企业职工劳保医疗时期
城镇企业职工医疗保障制度创立于新中国成立初期，由公费医疗和劳保医疗两个部分组成。机关、事业单位的工作人员、伤残军人和大学生是公费医疗的对象，其经费主要来源于各级政府的财政拨款；国有企业员工是劳保医疗的主要对象，其经费主要来源于在企业中按照一定的工资比例抽取的福利待遇。企业员工劳保医疗的制度曾一度为企业职工提供了医疗保障。但是随着我国经济体制改革的深入，这种制度的弊端也逐步暴露出来：单位对职工医疗费用包揽过多，造成企业经济上不堪重负甚至入不敷出的局面；医生和患者之间缺乏有效的制约机制，医药费用呈现大幅度不合理增长，浪费严重；劳保医疗制度覆盖面狭窄，受众范围过小，无法将福利惠及全民等一系列问题。
