摘要：目的：掌握崇明区的慢性病管理状况，提高居民的健康素养水平。方法：本研究通过文献研究法，了解并分析国内外不同的慢性病管理模式的概念和内容，对崇明区社区卫生服务中心的慢病患者进行问卷调查，对医务人员进行访谈研究。结果：结果显示崇明区慢性病管理模式中存在主要问题有：1、社区居民自我健康意识不足；2、慢性病管理项目与常规工作脱节；3、软硬件投入不够，缺少必要的检查设备及药品。结论：慢性病分级管理和自我管理模式有助于减少医疗费用支出，本文从政府、社区卫生服务中心和慢病患者三个层面给出了相应的建议，应加强慢性病健康知识教育，完善政策的落实，提高居民自我健康意识。78199

Study on the Management Model of Chronic Diseases in Community Health Center of Chongming District, Shanghai 

Abstract: Objective:To understand the management of chronic diseases in Chongming District, improve the health literacy level of residents。Methods:Through the literature research, to understand and analyze the concept of different chronic diseases and management at home and abroad the concept and content。The questionnaire survey was conducted on the patients with chronic diseases in the community health service center of Chongming District, and Interviewed with medical staff。Results:The results showed that Chongming District, chronic disease management model in the main problems are:1。Community residents lack of self - health awareness;2。Chronic disease management programs are out of touch with routine work;3。Hardware and software investment is not enough, the lack of necessary inspection equipment and drugs。Conclusions:Chronic disease classification management and self-management models help reduce health expenditure。This article from the government, community health service centers and patients with chronic disease three levels given the corresponding recommendations。Health education for chronic diseases should be strengthened, the implementation of policies and the health awareness of residents should be improved。
