摘    要

目的：通过调查嘉兴市城区居民社区卫生服务利用现状，分析其主要影响因素并提出提高该地区居民对社区卫生服务利用的建议。方法：通过自行设计的调查问卷，采用简单随机抽样的方法对嘉兴市2个主城区（南湖区和秀洲区）的6个社区的1227名居民进行调查。采用描述性统计对居民社区卫生服务利用进行现况研究，采用c2检验和二元logistic回归分析影响居民社区卫生服务利用的因素。结果：本次调查共发放问卷1240份，回收有效问卷1227份，有效回收率为98。9%。嘉兴市居民社区卫生服务总利用率达78。65%（（利用过人数/调查人数）。居民获得的社区卫生服务列在前三位的项目分别是开药、看病就诊、健康咨询。疾病康复、预约转诊、家庭护理、预防保健、家庭保健员培训等项目的利用率均不足20%。居民首选社区卫生服务机构就诊的比例为64。31%。离家近、看病取药方便、医生服务态度好是居民首先选择社区卫生服务机构就诊的前三位原因。居民两周患病率为31。54 %，两周患病社区就诊率为58。66 %。以“居民两周患病社区就诊率”作为衡量社区卫生服务利用的指标，logistic回归结果显示家庭人均月收入（OR=1。167,P=0。009）、每年看病所花费的金额中用在社区卫生服务机构的比例（OR=0。708,P＜0。001）、卫生服务可及性（OR=1。290,P=0。017）、对社区卫生服务的知晓情况（OR=0。704,P=0。018）是影响居民社区卫生服务利用的关键因素。结论：嘉兴市居民对社区卫生服务的利用率较高，但仍以看病就诊开药为主，比较单一，利用的程度不高。社区卫生服务机构应进一步发挥其居于社区、方便便捷的优势; 加强社区医疗团队的建设，增加居民的信任感；同时加强社区卫生服务内容的普及与宣传，密切与社区居民的联系，从而进一步提高社区卫生服务的利用水平。85384
